Set up of password entry portfolio

Online & telephone support for one year

Cost £1000
If portfolio satisfactorily completed in six months Level One Assessment visit included.

Level One Assessment

Cost £500

Portfolio and Patient Friendly Team Support £50 per month until Recognition assessment.
Invoiced quarterly in advance.

Recognition Assessment visit  £1250.

Re-assessment visit after one year £650.

Maintenance of portfolio £100 per year thereafter.

Optional workshop at any stage of process
For the whole staff team or representatives of each element – clinical, managerial and administrative.
Optimum timing 10am-4pm.
Essence can be delivered in 3 hours.
Two facilitators direct proceedings.

Cost  £1000
This covers up to 30 delegates’ educational materials and facilitated learning.

Travel and other expenses for facilitators may be additional.

Costs of venue, equipment, refreshments, reimbursement of expenses, locum and backfill costs for participants to be borne by the commissioning location.
